New Patients & Scheduling
During your first visit, a comprehensive interview and examination will take place to diagnose your dental concerns, review your past medical and dental history, and create a treatment plan that provides for your optimal dental health. Digital radiographs (x-rays) are usually taken at this time to check for cavities, fractures, tumors, and infections.
- Please arrive 15 minutes before your initial appointment time to fill out new patient forms.
- Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
- Insurance information is collected before you come in so benefits can be checked in order to answer any insurance questions you may have at the time of service.
- If recent x-rays were taken, please email them prior to your appointment date.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
